New Patient Registration
All New Patients must have a New Patient Registration form filled out and on file with us at Crosswinds Animal Hospital. By signing and submitting this form you agree to pay any fees and charges associated with exams and treatments for your pet.
Also, all clients who fail to show will be considered a No Show and will be charged a $50 No Show Fee.
All New Clients will be charged a $50 fee prior to scheduling an appointment with a doctor. This fee will be applied to the account and will go towards your first appointment.
In the event a new client fails to show, the prepaid fee is non-refundable and you will be required to prepay an office visit again prior to rescheduling.
